Strength-Based Assessment Form

The strength-based assessment form helps to guide the transition process based on strengths, interests and desires for the future. There is a parent section and a student section. Many times the student and the parent may have different dreams for the future and these dreams can change year to year. It helps to open discussion about the future and to guide the transition meeting, the transition section of the IEP and transition services for future success after high school.

Positive Student Profile

A Positive Student Profile is an excellent way to “introduce” the child to new teachers and to others. The profile provides you with the opportunity to see a child’s strengths and challenges in a new light, have a clearer understanding of who the child is a become better prepared to participate in the IEP process. A new profile can be completed each year as the child grows and changes.
